Mesothelioma is an uncommon disease that causes malignant cancer cells to form within the lining of the chest, abdomen, or around the heart. Its primary cause is believed to be exposure to asbestos.

Malignant mesothelioma is also known as asbestos cancer or simply "meso." Mesothelioma causes cancerous cells to develop in the body's mesothelium, where they can spread to and damage vital organs and tissue. These malignant cells can also metastasize to other regions of the body. Mesothelioma is very difficult to diagnose and responds poorly to most treatment modalities, accounting for a poor prognosis.
The disease derives its name from the mesothelium, a sac-like membrane that protects most of the body's internal organs. It is divided into two distinct protective layers of cells: the visceral (the layer directly surrounding the organ) and the parietal (a sac around the body cavity). By releasing a lubricating fluid, the mesothelium allows the organs to move more freely within the body cavity; for example, the contraction and expansion of the lungs. The mesothelium is also referred to according to where it is located in the body: pleura (chest), peritoneum (abdomen), and pericardium (heart).
Over two-thirds of all mesothelioma cases begin in the pleura region. Pleural mesothelioma spreads through the chest cavity, occasionally developing in the lungs as well. The disease most commonly causes pleural effusion, an excess build-up of fluid inside the chest cavity. This excess fluid increases pressure on the lungs and restricts breathing. In addition, malignant cells can cause the pleural lining to thicken and restrict the breathing space even further.
Peritoneal mesothelioma is the second most common form of the disease, accounting for less than 30% of all cases. Malignant cells form in the peritoneum, affecting the abdomen, bowel, liver, and spleen. Similar to pleural mesothelioma, the disease also causes a build up of excess fluid in the abdominal cavity. Normal bodily functions, such as digestion, can be hindered by the obstruction of organ movement.
Very rare forms of mesothelioma occur in the pericardium, as well as the mesothelium of the male and female reproductive organs. Cystic mesothelioma of the peritoneum, another rare form of the disease, occurs predominantly in women and is more benign in nature.
Malignant mesothelioma takes the form of one of three cell-types: epithelioid (50% to 70% of cases), sarcomatous (7% to 20% of cases), and biphasic/mixed (20% to 35% of cases). Of these cell-types, epithelioid mesothelioma carries the most favorable prognosis, followed by biphasic, and finally sarcomatous (very aggressive).
Mesothelioma remains relatively uncommon in the United States, with approximately 2,500 new cases reported annually. The incidence rates are much higher in Western Europe (over 5,000 cases reported annually). These numbers are expended to climb dramatically over the next 20 years. Older males (median age 60 at diagnosis) are three to five times more likely to develop mesothelioma than women. This is most like do to male predominance in those professions with an increased risk of asbestos exposure.
Approximately 80% of all mesothelioma patients have a history of asbestos exposure. The majority of these patients were employed in an industry that involved the use of asbestos in some fashion. In addition to occupational exposure, household exposure of family members is not uncommon. An exposed individual can carry the asbestos particles on their clothing, skin, and in their hair when they return home, resulting in paraoccupational exposure. Even brief exposure to asbestos, as little as one to two months, can result in long-term consequences. Although the dangers of asbestos have been known for decades, the long latency period of mesothelioma (30 to 40 years) means that majority of patients were already exposed as far back as the 1950s. Malignant Mesothelioma is a rare form of cancer that affects the thin cell wall lining of the body's internal organs and structures. This lining is known as the mesothelium. Malignant mesothelioma has three known varieties. They are malignant pleural mesothelioma, malignant pericardial mesothelioma, and malignant peritoneal mesothelioma. Pleural mesothelioma occurs in the pleura, the lung's lining. Peritoneal mesothelioma occurs in the peritoneum, the abdominal cavity wall. And pericardial mesothelioma occurs in the lining of the hearth, known as the pericardium.

Mesothelioma is known only to be caused by asbestos exposure. Asbestos is a nature, yet toxic mineral that was used commonly in heavy industry. Microscopic asbestos fibers enter the body through the lung or ingestion. Once inside, the durable fibers are unable to be broken down or expelled by the body, causing a harmful inflammation and scarring of the mesothelium. This scarring lays the groundwork for malignant mesothelioma and other respiratory conditions, such as asbestosis.

There is a great deal of latency associated with asbestos exposure and the onset of mesothelioma. Often patients will not encounter mesothelioma symptoms until 30-50 years following exposure to asbestos. In many cases, those diagnosed with mesothelioma are not those who were directly exposed to asbestos in a jobsite setting. Spouses and children of those exposed to asbestos who frequently came into contact with the fibers on the clothing or person of those exposed have also developed malignant mesothelioma as a result



Malignant mesothelioma prognosis is poor, as often the disease will be diagnosed in its later stages after symptoms have appeared. However, there are several treatment options for the management of the cancer. Among these are mesothelioma chemotherapy, radiation therapy, and surgical resection. Chemotherapeutic and radiation therapy methods are more likely but surgery is often an option for those whose disease is diagnosed early enough.

More aggressive therapies are now possible for patients with mesothelioma diagnosed in its earlier stages. For instance, extrapleural pneumonectomy is now possible in many pleural mesothelioma patients who are deemed eligible for aggressive surgery. Extrapleural pneumonectomy involves the removal of the entire affected lung, the pericardium, the pleura, and the diaphragm. This aggressive surgery is performed frequently in early stage mesothelioma patients by Dr. David Sugarbaker at the Brigham and Women's Hospital in Boston, MA.


Patient's who are eligible to receive an aggressive surgery, like an extrapleural pneumonectomy, will have a far more favorable prognosis than those diagnosed with advanced stage inoperable disease. However, there are treatment options for nearly all mesothelioma patients. Chemotherapy cocktails such as the use of Alimta® in conjunction with Cisplatin, as well as numerous other clinical trial being carried out give mesothelioma patients the ability to slow the advance of the disease and extend survival rates.

Malignant pleural mesothelioma is the most common type of mesothelioma and occurs in the mesothelial membrane surrounding the chest cavity and lungs. Pleural mesothelioma, because it is more common than other varieties of the malignancy, has had more research invested in it. Pleural mesothelioma represents approximately 70% of all mesothelioma diagnoses.Malignant peritoneal mesothelioma is less common than pleural mesothelioma and represents approximately 15-20% of all mesothelioma diagnoses. Peritoneal mesothelioma occurs in the lining of the abdominal cavity in the membrane surrounding the organs of this region. Like pleural mesothelioma, peritoneal mesothelioma is known only to be caused by exposure to asbestos.

Malignant pericardial mesothelioma is among the rarest of all known malignancies, and represents less than 10% of all mesothelioma diagnoses. Pericardial mesotheliomas occur within the mesothelial lining of the heart. Pericardial mesotheliomas, like those of pleural and peritoneal varieties, are associated with asbestos exposure. Pericardial mesotheliomas differ from the pleural variant in that they are known to occur in younger individuals. While surgery is seen as the preferred method of treatment, mobidity and mortality are almost certain given the region in which the tumor is located.

A pleurectomy / decortication (P/D) is an operation for mesothelioma that removes the involved pleura and frees the underlying lung so that it can expand and fill the pleural cavity.

The pleural space is a potential cavity between the lung and the chest wall-more specifically, between the visceral pleura and the parietal pleura. In the average healthy patient, this space is less than 1 mm thick. There are a number of pathologic processes that can alter the transport of cells and fluid within this space and thus give rise to clinically significant sequelae.

One such process is fibrothorax, which is defined as the presence of abnormal fibrous tissue within the pleural space, resulting in entrapment of the underlying pulmonary parenchyma (a state variously referred to as trapped lung, restrictive pleurisy, or encased lung).

Decortication is the surgical procedure by which this restrictive fibrous layer is peeled away from the lung; the literal meaning of the term is the stripping away of a rind. The technical goals of the operation are to re-expand the lung and resolve the pathologic process affecting the pleural space so that pulmonary function and chest wall mechanics will improve and the patient's symptoms will be relieved.
